PRIVACY CONSENT CLAUSE

PRIVACY CONSENT TO COLLECT, USE AND DISCLOSE THE CONFIDENTIAL INFORMATION PROVIDED BY YOU IN THIS APPLICATION: The confidential information provided by you is protected by Canadian Privacy Legislation called PIPEDA. You must consent to the disclosure and use of this information in order to register with the London Junior Knights. The purposes behind collecting, using and disclosing tryout information is set out in the London Junior Knights website www.londonjuniorknights.com . The organization, its coaches and volunteers will use your information. It will not be used or disclosed to anyone else without your consent. By signing and submitting this application the player, his parents and/or guardians, are collectively granting their consent.

1. Opt-Out Provision

I acknowledge and understand that, for the purposes of offering additional products and services that may be of interest to the participant or the participant's family, London Jr. Knights may use or disclose the participant's name and address collected on this form to third parties. As the above-mentioned participant, or the participant's legal guardian, if you do not wish that information used or disclosed for the purpose of offering you such additional products and services. 

2. I, the undersigned certify the above information to be true and in consideration of the granting of this application to me with the privileges incident thereto, and by signing this application I have become subject to the rules, regulations and decisions of H.C, its Board of Directors, its branches and/or divisions which may be restrictive in some areas such as movement from team to team, conduct etc. and I agree to abide by such rules, regulations and decisions of H.C , its Board of Directors, its branches and/or divisions. I am aware that these rules and regulations are available to me through my team manager.

TRYOUTS AND AFFILIATION

In fairness to both participants and coaching staff during the selection process, if an individual participant does not wish to play AAA but wishes to play AA, they have the option of indicating their preferred level prior to tryouts. This will ensure all participants have the opportunity to play at their preferred level. (this line s/b removed) Participants are expected to play at their selected level. If a participant refuses to play at their selected level, once they have indicated they would play, the participant may be refused membership in our program and their fee refunded, less expenses.

AFFILIATION: If my child was to participate on a London Junior Knights team I hereby give my permission for my child to be an affiliated player with another London Junior Knights team. I also fully understand that my child can only be affiliated with one team. This affiliation would be at a higher level or up one age group.

PARENTAL CONSENT TO PLAY : I, the undersigned parent or legal guardian of the above named player, do hereby consent and agree that the above named minor may participate in the sports programs of the London Rep Hockey Inc. operating as the London Junior Knights. It is agreed that the named Association or their Sponsors assume no responsibility for injury as a result of such participation or in traveling to or from such participation. It is further agreed that all rules and regulations will be abided by, both the participants and the parents. Please be advised that it is illegal for anyone to play for the London Junior Knights and any non-CHA Team. I understand that anyone found playing illegally would be suspended for the season, without refund.

CODE OF CONDUCT PLAYER / PARENT / LEGAL GUARDIAN

Membership in the London Rep Hockey Inc., hereinafter called the London Junior Knights, is not a right, it is a privilege. Members must abide by the Constitution, Policies and Procedures and their behavior is expected to mirror the spirit of the Code.

All members of the London Junior Knights shall respect other H.C., O.H.F., O.H.L., LONDON JR. A KNIGHTS, ALLIANCE, and LONDON JUNIOR KNIGHTS members, officials, parents, players, team officials, volunteers and Board of Directors. Any inappropriate conduct harassment or abuse (includes profanity) directed towards game or team officials, parents, players, volunteers or Board of Directors of the C.H.A., O.H.F., O.H.L., LONDON JR. A KNIGHTS, ALLIANCE , and LONDON JUNIOR KNIGHTS will not be tolerated.

All members of the London Junior Knights shall respect the game of hockey and shall behave in a manner so as not to make a travesty of the game.

Violation by any member of any provision of the Code of Conduct may result in an immediate and indefinite family (including player) suspension or expulsion from the London Junior Knights. In the event of an alleged violation of the Code of Conduct, the Discipline Committee shall be notified and a resolution attained as soon as practical. The Discipline Committee's decision is final.

I have read the above Code of Conduct and agree with it. I realize that any violation of the Code of Conduct may result in suspension or expulsion from the London Junior Knights of myself or my family.
